Student Athlete Medical Clearance Information

The following instructions must be followed by all those interested in trying out for a varsity intercollegiate team at the University of Massachusetts Dartmouth. All varsity student-athletes are required to complete all pre-participation medical prerequisites of the University of Massachusetts Dartmouth Sports Medicine Department PRIOR to being allowed to participate in any practice, game or conditioning session with their respective athletic programs. Read and follow the directions carefully as each portion requires special attention and contains important information for student-athletes.  These steps need to be completed and approved annually by the Sports Medicine staff.  During this review the Sports Medicine staff may require additional documentation associated with any medical conditions or recent injuries, surgeries or treatments you may have experienced.

You must contact your team head coach directly to be verify your access to the Sports Medicine medical clearance documentation.  Only those who are verified by the team head coach will be reviewed by the Sports Medicine staff. 

Below you will find information and web links which will assist you in preparing the necessary documentation required to receive medical clearance to compete as a varsity athlete at UMass Dartmouth this season.  Please note that medical paperwork will only be accepted Monday through Friday from 10AM to 1PM once classes resume.  These forms can be submitted to the Sports Medicine Staff by email, uploading documents into the ATS database found on the link below or in person.

The following checklists have been developed to aid student athletes with all the requirements that will need for participation in intercollegiate athletics for the University of Massachusetts Dartmouth.  Your coach will be updated with all this information.  If you have any questions regarding your status please reach out to them directly.

PLEASE NOTE - these documents are different from those needed by University of Massachusetts Dartmouth's Health Services. The below documents should only be used for medical clearance to participate as an UMass Dartmouth varsity student-athlete!

Athletic Injury Secondary Insurance Policy

The University of Massachusetts Dartmouth participates in a secondary insurance policy intended to relieve some of the financial burden caused by injuries and select illnesses caused while participating in team related activities as a student athlete.  Please read the following letter regarding the coverage provided as well as expectations of the student athletes to qualify for this service.

Medical Clearance Procedures For 2025-26 Seasons

First Year Student Athlete Information (including any transfers)
 

1. Physical exam within 6 months of your first day of participation UMass Dartmouth Sports Medicine form should be used whenever possible. Form should be uploaded into your ATS portal as a .pdf. (Click here for form)
 
2. Complete all required documents on ATS website (CLICK HERE FOR WEBSITE)

    a. Profile created including insurance and emergency contact information
    b. Complete the questions on the medical history tab
    c. Complete the documents on the forms tab

3. Submit required documentation associated with sickle cell status.  Forms should be uploaded into your ATS portal as a .pdf.

4. Complete and submit the Health Services Waiver.  All highlighted sections must be completed.  Form should be uploaded into your ATS profile.

5. Medical Exemptions: Must be submitted and approved before allowed to participate.  The NCAA recognizes that some banned substances are used for legitimate medical purposes. Accordingly, the NCAA allows exception to be made for those student-athletes with a documented medical history demonstrating the need for treatment with certain banned medications. Medical exceptions may be granted for substances in the following banned drug classes: anabolic agents*, stimulants, beta blockers, diuretics and masking agents, hormone and metabolic modulators*, beta-2 agonists, peptide hormones*, growth factors or related substances and mimetics*, and narcotics.

If necessary, submit any required documents for medical exemptions.  Exemptions are only needed if being currently medicated for the following conditions.  Forms should be uploaded into your ATS profile.

6. Complete the Concussion Vital Signs Baseline testing.

Returning Student Athlete Information
 
1. Physical exam within 1 year of the first date of preseason activities (8/12/25) participation UMass Dartmouth Sports Medicine form should be used whenever possible. Form should be uploaded into your ATS portal as a .pdf. (Click here for form)

This does not need to be completed yearly.  Only new prescriptions will need a medical exemption.

Completion of Medical Paperwork

Once you have completed, and/or reviewed all medical clearance documentation, please upload these onto the ATS online database.
